Xion appears before Sora in a dream during his forced second sleep in the World That Never Was. Sora first sees Naminé, who turns away as he tries to thank her. When Sora grabs her wrist she turns around into Xion. Sora doesn't seem to know her, but still he sheds a tear. As Xion turns away once again the image of her face disappears, suddenly covered by the hood of her black coat. She runs away from him. By the time, Sora catches up to her again, it's not her at all; it's Roxas.
After Sora catches up to Naminé in his dream in the World that Never Was, she suddenly transforms into Xion. Although Sora doesn't recognize her, he suddenly sheds a tear. Xion raises her hood and runs off.

Something about her existence (or lack thereof) sticks with him, however, as he later remarks to Xigbar that Axel, Roxas, Naminé, and "that other girl" all must've had hearts, because they could cry. Xemnas suddenly appears to confirm that yes, many members of the Organization had begun to develop hearts, but that the Organization's true goal had been to fill the Nobodies' empty bodies with copies of Xehanort's heart. Outraged by this disregard for the value of a heart, Sora stands up to Xemnas and Xigbar. When Xigbar questions the strength of Sora's heart, Sora doesn't waver. Xion momentarily appears, Keyblade in hand, alongside all of the other friends in Sora's heart, to support Sora. The immense power unnerves Xigbar, who flees, leaving Sora to a battle with Xemnas.
After catching up to Xigbar, Sora comments on how Axel, Roxas, Naminé, and "that other girl" all must have had hearts, saying "You need a heart to cry." When Sora reaffirms to Xigbar that his strength comes from his friends, Xigbar sees an image of the many individuals connected to Sora standing behind him, including Xion. Unnerved, Xigbar leaves Sora to Xemnas and leaves.

Riku later encounters Xion when he dives into Sora's heart, in the version of the Destiny Islands there. She serves as the last of three guardians after Roxas and Ventus. After answering questions for Roxas and Ventus, Riku sees Sora sitting on a paopu tree facing the sea. However, when he arrives at the tree, Riku discovers Xion there instead, gazing emptily at the sunset. He doesn't know her, but she knows him. "Riku," she addresses him, eyes still vacantly watching the horizon, "What do you wish?" "More questions," he observes exasperatedly, but he seems to realize that his answer is important somehow. He considers the question seriously for a moment before answering. "I wish..." Upon hearing his answer, Xion vanishes, just as Roxas and Ventus had before her, triggering Sora's awakening in the real world.
After Riku dives into Sora's heart and defeats the Armored Ventus Nightmare, he appears on a memory of the Destiny Islands. After meeting Roxas and Ventus, who each ask him a question, Riku finds Xion sitting on the Paopu tree. She asks him what he wishes, and disappears once he answers. Sora then awakens in the real world.

Riku then encounters Ansem the Wise, or rather, a data version of him planted inside Sora's heart by the real Ansem the Wise. He remarks that Sora's heart is special--like a child's--and that perhaps it would be able to restore existence to those thought lost forever. All Sora would need to do is be himself, and follow his heart wherever it may lead him.

Xion appears during the opening sequence, threatening Roxas with her Kingdom Key as he lands upon a stained glass window. Immediately the scene switches to the three of them—Axel, Roxas, and Xion—messing around, smiling, and eating ice cream atop the Twilight Town clock tower. The scene returns to Xion and Roxas, but Xion disappears in a burst of light, Keyblade and all. Suddenly, Roxas is surrounded by flames, and he pulls out the Oathkeeper and Oblivion, rushing into a battle with Axel.

As Roxas's Replica, Xion can wield the Keyblade. She wields her weapon fairly skillfully, being able to disarm Axel. Because Xion and Roxas are both part of Sora, they can both wield his Keyblades, even at the same time. Because their weapons are the same, the Gears produce the same forms when applied to either Keyblade.

It is notable that, when Xion runs with her Keyblade, she grasps it with two hands instead of slinging it over her shoulder like Roxas does. This is identical to how Sora runs with his Keyblade in Kingdom Hearts II.
