Where in the game does it say that she was revived by the forces of darkness? Wouldn't it be more accurate to say that she was revived by an unknown method or something like that?

It is heavily implied from the discussion between Pete and Maleficent in Villain's Vale that Maleficent herself planned to revive the fallen villains. Darkness brought Oogie Boogie back, no doubt, and that same darkness brought Maleficent back. We can safely assume that darkness brought Ursula back. Jafar and Hades are exceptional : Hades is immortal, and Jafar was trapped in his lamp.
