Okay, the Japanese sites I've looked on only list two Mushroom weapons per Mushroom, plus the Premium and Limited, while the Gamefaqs boards claim each mushroom gives all four of either shield or staff.
I did find one Japanese website which seems to say that the shield or staff you get depends on your score, but it was kind of hard to make out what it was saying.
Can anyone with Final Mix verify whether I have the drop lists correct?

I've played Final Mix and yes, they drop all four depending on the score. After breaking my record for Mushrooms #2 and #5 several times, I must say Gamefaqs have a point. I had already received three Mushroom shields from Mushroom #2 - as far as I can remember...
The Japanese sites also have a point : only that, those are the most frequently dropped shields/staffs for the respective Mushrooms. (Darn, I've sold tons of them.)
